Insurance FAQs:

Are you a participating provider in any health insurance plans?

No. We do not accept insurance. It is best to call your insurance company before making an appointment so that you can fully understand what the cost will be to you. 

Some questions that may be helpful to ask:

1. Is there a cap (maximum amount) you will pay for an office visit/given year?

2. Do I have a deductible I must first meet? If so, how much is that?

3. Do I need preauthorization? If so, can you grant me preauthorization to see this provider with the information I just gave you?

4. Is there a specific claim form I must use to submit claims and if so, where can I access that?

5. How much time after a doctor’s visit do I have to submit a claim?

6. Are there certain CPT Codes (Current Procedural Terminology Codes) that you do or do not reimburse for? Any that require preauthorization (for example, 90837)?

7. Do you currently reimburse for telehealth? Should those visits be coded in any specific way?

CPT 90791: Initial Intake

CPT 96136: Psychological test administration/scoring by qualified health professional first 30 minutes” + 96137  each additional 30 minutes 

CPT 96130,  + 96131 each additional 1 hour 

90847: Family Therapy with Child Present

90846: Family Therapy without Child Present

90832: Individual Therapy 30 minutes

90834: Individual Therapy 45 minutes

90837: Individual Therapy 60 minutes

Note that there will be the added codes (95-2) for telehealth.

Will you provide Superbills to submit to my Insurance for Reimbursement? We try hard to meet the needs of all patients. While none of the doctors accept insurance or Medicare, we will provide any necessary paperwork to submit to your insurance company if you pursue out of network benefits. While insurance companies may provide reimbursement, as is the case with any out-of-network psychologist, there are no guarantees for reimbursement. If your insurance denies reimbursement and working with us becomes cost-prohibitive, please inquire about our sliding scale policy. Superbills are provided to all of our patients on a monthly basis. 

Are you a Medicare participating provider? No. We do not accept Medicare. If you are currently enrolled in Medicare and wish to be seen by us, we can enter into a private contract in which you will be charged regular fees. We can provide you with a copy of the Medicare opt-out affidavit upon request.

The First Step with Us

We first meet with families for a 60 minute intake in order to determine which level of service is the best fit for you and how we can help you most effectively. After the intake, there is a follow up 45 minute feedback session. The total cost for these two meetings is $950.

Assessment Tiers & Fee Structure

We offer three levels of service to best meet your needs.

If you are unsure which tier is best for you, we can suggest what we think is most appropriate to your needs after briefly speaking on the phone. If you prefer, we can meet with you for a 90 minute consultation to start to understand what your family has been through, recommend next steps, and help you understand more about our process.

$1750 Tier 1

Consultation + Feedback

  • Intake +30 min feedback and a brief plan which includes recommended psychoeducational materials.

$4750 Tier 2

Communication + Support

  • Everything in Tier 1

  • Speaking with outside Professionals & School. 

  • Creating a detailed plan and initiating school, and therapy, and home coordination.

$7500 Tier 3

Comprehensive Coordination+Resources

  • Everything in Tier 2

  • Detailed plans for multiple routines and help executing initial steps.

  • Comprehensive School Coordination and Collaboration with Professionals.

  • Development and help with execution of Behavior Plan and Attendance Monitoring.

  • Psychoeducational Videos to accompany recommended education materials.

 

Consultation and Ongoing Coordination

One time consultation:

Dr. Miller and Dr. Sachs can make themselves available to speak with professionals or family members upon request for a one time meeting.. Please contact us to discuss details and fees.

Ongoing Coordination and Consultation:

After you have a clear understanding of your child’s needs and struggles and a concrete plan, your family may require someone to lead ongoing coordination with school staff, therapy professionals, and what is happening at home. This can include weekly meetings with relevant stakeholders as well as with your family. We are happy to discuss how to tailor this to your needs. Starting at 4 hours a month with Dr. Miller and Dr. Sachs ($2000), fees will range based on the level of coordination you require and who is part of the provider team.

Short Term Consultation with Outside Providers:

We welcome consulting with your established therapist who is an expert in treating your child. In our short-term consultation work, we will bring our targeted expertise in school refusal and avoidance behaviors to optimize your child’s treatment plan. The Consultation process will be individualized to meet your exact needs and will be billed by the hour ($540).

Intensive Therapy Service

Starting at 6 hours a week, fees will range based on who is part of the provider team.

 

Parenting Bootcamp and Ongoing Supports

Fees will range based on who is part of the provider team.

Parenting Bootcamp includes:

  • Understanding the “school refusal trap” and how to respond differently

  • Skills to remain calm, in control and thus able to use consistent strategies and access resources

  • How to guide your child's behaviors through routines and meaningful reward systems, all tailored to your child's specific underlying struggles.
